1. Comprehensive Skills Assessment: The journey begins with a 4-week comprehensive skills assessment. This allows our team to gain a thorough understanding of each participant's strengths, needs, and interests.
2. Collaborative Goal Development: Following the assessment, we dedicate the first week of the following month to working closely with the individual to develop personalized goals. These goals are designed to foster growth and independence in five key areas:
3. Family Partnership: We believe that family involvement is crucial for success. During the second week, we meet with families to discuss the proposed goals, ensuring they align with their priorities and provide us with valuable insights.
4. Individualized Program Plan (IPP) Development: The third and fourth weeks are spent creating a detailed Individualized Program Plan (IPP). This plan incorporates the agreed-upon goals, specific strategies, and measurable objectives that will guide the individual's progress.
5. IPP Implementation: Once the IPP is finalized and approved, we begin putting the plan into action during the first week of the following month. Our dedicated team provides ongoing support and monitors progress to ensure that each individual is making meaningful gains.
